Within NHS Wales, shared care represents circumstances where primary care accept the transfer of prescribing responsibility from specialist care, for medicines that require long-term regular monitoring. The monitoring arrangements and responsibilities are set out in a formal shared care protocol for the specific treatment, and the patient continues to be followed-up in reviews by the specialist. This document updates the principles of best practice for shared care prescribing agreements.
