Yellow Card Centre Wales Annual Report 2025 - 2026

Yellow Card Centre Wales (YCC Wales) annual report shows the number of Yellow Card reports for suspected adverse reactions to medicines submitted in Wales increased by 7% in 2025 - 2026.

A total of 4,018 reports were submitted during the year, compared with 3,757 in 2024 - 2025 and for the first time YCC Wales also received reporting data relating to medical devices.

Cardiff and Vale University Health Board recorded the largest percentage increase, with reporting rising by 34%, while Swansea Bay University Health Board saw a 16% increase. Reporting also increased across Betsi Cadwaladr, Cwm Taf Morgannwg, Hywel Dda and Powys Teaching health boards. 

Patients, parents, carers and consumers continued to make the largest contribution to reporting, accounting for 28% (1,126) of all reports. General practitioners were the second largest reporting group, submitting 799 reports (19.9%). 

The report also highlights increases in reporting from community pharmacists, GPs, nurses and trainee pharmacists, alongside a significant increase in reports from medical students.

YCC Wales continued to work closely with Yellow Card Champions, Welsh Medication Safety Officers (MSOs) and members of the Welsh Medicines Safety Network throughout 2025 - 2026.

The team also strengthened its links with universities across Wales, providing teaching on Yellow Card reporting to pharmacy and medical undergraduates at Cardiff and Swansea universities, and engaging with pharmacy students at Bangor University for the first time.

A range of training sessions were delivered to healthcare professionals and patients, supported by bilingual promotional materials for use in primary and secondary care. YCC Wales’ mascot, Yellow Card Charlie, also continued to feature in awareness activities, alongside social media activity to encourage reporting.

In July 2025, YCC Wales worked with AWTTC to bring colleagues together for a joint Medicines Safety Training Day and AWTTC Best Practice Day, recognising the contribution of Yellow Card Champions and healthcare professionals involved in medicines safety.

Looking ahead, YCC Wales will continue to develop new initiatives to encourage reporting of suspected adverse drug reactions and medical device incidents through the Yellow Card Scheme, with the aim of further increasing reporting across Wales.
