This guidance was developed through collaboration with partners in both health and social care and outlines standards on collaborative governance to support safe and optimised medicines management, for a consistent and reliable approach to medicines management in community care in Wales. The overarching goal is to ensure that each person receives the highest standard of medicines management tailored to their specific needs.
