Wales General Ophthalmic Services – Signed Orders Formulary

This document was developed in collaboration with Wales General Ophthalmic Services (WGOS) National Clinical Leads. WGOS enable all optometrists included in the NHS Wales Combined List (Supplementary Ophthalmic List) to offer NHS-funded sight tests and examinations for urgent eye problems. Typically, these include patients with dry eye, acute infection and allergic eye conditions. From April 2026, optometrists will be able to order the medicines or devices required for the treatment of those conditions under the NHS using a signed order. Community pharmacies will be able to supply NHS-funded medicines ordered by optometrists in Wales on a standardised NHS signed order form.

This formulary provides clinical information on eye preparations, including indications and product types, to support WGOS optometrists to work safely and consistently within national guidance. It also includes good‑practice recommendations to enable optometrists to offer effective advice before writing a signed order.  Members of the public, optometrists, pharmacists, GPs and other health professionals can view the related information on NHS‑funded WGOS eye care services in Wales on the Eye Care Wales website.
